Can you show what you've got in DesignModeler?
Poor element quality in this sort of system is typically associated with large jumps in cell size over small gaps or poor chosen sizing on a narrow surface. If you use the Mesh Metrics you can plot the quality & highlighting a column will show where these cells are.
