  Is there some new workflow in the site that requires Moderators to Approve posts?
  
  I saw this and there was a post waiting for approval, which I approved.
  
  It resulted in this Post.
  
  Did this get flagged by the spam filter because the username is "blank"?

  Strange. I looked for permissions defined for each role but there is no such approval required. Let me know if you see such message again for approval. The spam filter is ON and word "Blank" is allowed.
